Main Use Active Ingredient Marketed Name
Hypertension, fluid retention or hirsutism Spironolactone Spiractin

Uses
Spiractin (Spironolactone) is indicated for high blood pressure and fluid retention (oedema). It works by triggering an increase in the amounts of water and sodium released in the urine. This medicine is categorized as a potassium sparing diuretic, meaning that it helps retain potassium in the body, even though other substances are excreted. Its mechanism of action is achieved by attaching to certain receptors in the body and inhibiting the activity of a hormone called aldosterone.

The result of this is that the amount of salt and water removed is increased, while the potassium levels are preserved, thereby preventing potassium deficiency (which commonly occurs if other types of diuretics are used). It is commonly used to manage the symptoms of oedema (excess accumulation of fluid in the body), especially in patients with cirrhosis of the liver, nephrotic syndrome and congestive heart failure. In cases of hypertension that are not effectively controlled with other medicines, it may be used on its own (or combined with other medicines) to help control symptoms.

Another effect of this medicine is that it can inhibit the production of the hormone androgen. It has therefore been used to treat and hirsutism (excess body and facial hair growth) in certain women, especially those suffering from polycystic ovary syndrome. Acne can also be treated with it. Primary hyperaldosteronism or Conn`s syndrome (a disorder which causes too much aldosterone to be produced) can be treated with this medicine, and the decreased aldosterone production caused by this medicine can help to alleviate high blood pressure and other symptoms caused by this condition.

Dosage and Administration
Spiractin (Spironolactone) tablets each contain 25mg of the active ingredient. The dosage needed for the treatment of oedema, high blood pressure or hyopkalaemia generally varies, and may range from 25mg to 200mg, taken as separate doses. Your doctor will determine the dosage on an individual basis, and will also periodically reassess your dosage so as to determine whether or not any changes to the schedule are needed. Other disorders which are not listed here may require a different amount of medicine.

Side effects
Spiractin (Spironolactone) may trigger some side effects:

  • Erectile problems
  • Abnormal bleeding of the vagina
  • Feeling thirsty and having a dry mouth
  • Vocal changes resulting in a deeper voice
  • Painful sensation or enlargement affecting the breasts

It is essential to notify your doctor of all side effects which you experience. Side effects of a less harmful nature are generally the most commonly occurring, however the possibility of severe reactions must not be ruled out. You will be required to see your doctor at once if you notice a drop in the frequency at which you pass urine, problems related to movement, muscular twitches, low energy levels, a significant reduction in appetite or any other side effects that have not been listed here.

Precautions
Your doctor will need to monitor you for signs of electrolyte imbalance or fluid problems. Patients may also be told to watch out for this themselves.

Doctors may recommend that patients administer the medicine earlier in the day, as if taken at night, the increase in urination caused may disrupt sleep.

Seek immediate emergency medical attention if you suffer an allergic reaction. Symptoms to watch for, which may be indicative of a reaction, include skin rashes, hives, swelling of the face or limbs, trouble breathing, and trouble swallowing.

Always use Spiractin (Spironolactone) as you have been prescribed by your doctor. Never self-medicate or change your dosage without first consulting your doctor. The correct dosage can vary depending on your health, medical history, and the severity of the condition being treated.

This medication may not be safe for all patients. Before you begin using it always disclose the following to your doctor:
